KELENTURAN ATRIOVENTRIKULAR PADA STENOSIS MITRAL

Abstract


Abstrak. Pada pasien Stenosis Mitral terjadinya peningkatan tekanan arteri pulmonal dan berat-ringannya gambaran klinis yang muncul tidak selalu berkaitan dengan area efektif katup mitral yang mengalami stenosis dan perbedaan tekanan transmitral tetapi dari beberapa penelitian yang ada menunjukkan bahwa net kelenturan atrioventrikular (Cn) juga sangat mempengaruhi. Kelenturan atrium menjadi penyangga (buffer) terhadap perubahan tekanan di atrium selama siklus jantung. Pasien dengan net kelenturan atrioventrikular yang rendah (< 4 ml/mmHg) memiliki tekanan sistolik arteri pulmonal yang lebih tinggi.

Abstract. The severity of symptom and increasing of  pulmonary arterial pressure in patients with mitral stenosis not only depent on mitral valve area and mitral valve gradients but also influenced by atrioventricular compliance. Atrial compliance can be buffer for alteration of intra atrial pressure. Systolic pulmonary arterial pressure in patients with atrioventricular compliance less than 4 mm/mmHg is higher than another.
